Visuchika Chikitsa

Introduction:

Owing to consumption of enormous amount of food, all three Dosha get aggravated. These aggravated Dosha mingle with undigested food, which remains in stomach in un- assimilated form.

This further leads to manifest typical pricking pain (like pricking by needle). So this complete clinical manifestation is called as Visoochikaa.(A.H.Su.8/7-8)

Severe reduction of digestive power is the principle component in Visoochikaa. So while treating Visoochikaa one should always focus on enhancement of Agni(Digestive power).

Avasthika Chikitsaa

Aggravated state of Visoochikaa It is advisable to perform Agnikarma at Paarshn`ee (Calcaneus region).

Patient should remain fasting for a day, followed by regular treatment protocol like in Virechana (Samsarjana Krama)

In Saadhya Visoochikaa, Paarshn`ee Daaha (i.e. Agnikarma at Calcaneus region) helps to arrest extensive diarrhea.

Aamavasthika Chikitsaa

Saampaachana – In Visoochikaa, Agnimandya is underline cause.

In Aamaavasthaa, Aama Paachana should be done.

Vamana — Aamaas`haya sthita Aamaanna Teekshn`a Vamana (with Madana Phalaadi Dravya )

Pakvaavasthaa

    • Treatment as per specific state.
    • Various modes of Langhana (fasting etc.) should be done.
    • Virechana should be done with prior Svedana (sudation).

  • S`hodhana (Vamana and Virechana) in Visoochikaa pacifies complications like Moorchchhaa, Atisaara etc.

  • Visht`ambhaavasthaa in Visoochikaa Nirooha Basti (S.U.56/12).

  • Pathyakaaraka Aahaara should be given after appropriate evacuation of Dosha and adequate Langhana (fasting).(S.U.56/19)

Visarpa Chikitsa

Introduction:

  • The specificity or characteristic of this disease is that it spreads like snakes venom (Sarpa Visha). (C.Chi.21/6)
  • It is a dreadful (Daarun`a) disease if neglected, hence should be dealt with immediately, (C.Chi.21/6-7)
  • In the treatment of Visarpa depending upon the Avasthaa the following treatment should be adopted.

Saamaanya Chikitsaa:

  • The first three types of Visarpa are curable, while Sannipaataja and Kshataja Visarpa are considered as incurable.
  • In Visarpa the best treatment is Rakta Visraavan`a. (S.Chi.17/16) Bahyopachara (A.H.Chi.18/10)
  • Blood letting should be followed by S`hodhana and then procedures such as Pradeha etc. should be adopted for S`hamana of Visarpa.
  • Based on the combination of the Dosha the treatment should be decided. (A.H.Chi.18/20)
  • If there is predominance of Vaata Dosha, then the Lepa should contain more of ghee. (A.H.Chi.18/18-19)
  • In all kinds of Visarpa, when they suppurate treatment should be similar to Vran`a. (S.Chi.17/17)

Summary of treatment:

  • Tikta Dravya Pradhaana Dravya should be used.
  • Among the various formulations advised; such of the formulations which have predominantly S`heeta Gun`a are for Pittaja Visarpa, Rooksha Gun`a for Kaphaja Visarpa, Snigdha Gun`a are for Vaataja Visarpa.
  • VaataPitta S`haamaka Chikitsaa for Agni Visarpa is helpful, similarly Kapha – Pitta S`haamaka Chikitsaa in Kardama Visarpa.(C.Chi.21/116-117). (A.H.Chi.18/18-19)

Aavasthika Chikitsaa:

  • In Kapha Sthaanagata Saama condition:

o Langhana

o Vamana

o Tikta Rasa Sevana and

o Rooks`ha S`heeta Lepa is adviced.(C.Chi.21/44).

Here Kapha Sthaanagata means Oordhva Kaayagata Visarpa.

  • In Pitta Sthaanagata Saama condition:

o Along with Langhana Tikta Rasa Sevana.

o Vishesha Chikitsaa advised is Rakta Mokshan`a and Virechana.

Here Pitta Sthaanagata means Madhya Kaayagata Visarpa. . (C.Chi.21/45).

  • In Vaata Sthaanagata Saama condition

o Rooks`han`a Chikitsaa is advised.

o Also Snehana is contraindicated in the initial stage of treatment if there is association (Anubandha) of Rakta or Pitta i.e, Rookshan`a Chikitsaa only has to be employed. . (C.Chi.21/46).

  • In Bahu Dosha Avastha:

o Medicated ghee that has the purgative action should be used. (The ghee to be used in Bahudoshavasthaa should essentially have Virechana action). If such ghee is not used then the Dosha_s become immovable or rigid (Sthabdha) and settle down in Tvacha, Rakta and Maamsa and bring about suppuration (Paaka) of the same. Hence Virechana should be advised in Visarpa. . (C.Chi.21/48).

o Raktamokshan`a is always advised during the initial stage, because blood (Rakta) being the main seat (Aas`hraya) of Visarpa, hence when vitiated blood is let out it pacifies the disease. (C.Chi21/48-49)

o Mahaatiktaka Ghrita of Kusht`ha and Traayamaan`a Ghrita of Gulma Adhikaara should be used.

S`haakahagata Visarpa Chikitsaa:

  • In this Avastha Raktamokshan`a is the treatment of choice as Rakta is mainly vitiated.(S`haakaha implies Raktaadi Dhaatu_s).
  • If Rakta is vitiated due to Vaata Dosha, then Raktamokshan`a should be done with Vishaan`a, if by Pitta Dosha then Jalouka should be used, if by Kapha Doshathen by Aalaabu or Siravedhana should be done at the nearest site of Visarpa.
  • If Raktamokshan`a if is not done early or at the right time then Rakta attains Kledatva, hence leading to the vitiation of the other Dhatu_s i.e. Tvak, Maamsa,Snaayu.(C.Chi.21/68-70) (A.H.Chi.18/8)

S`hamana Chikitsa:

After Proper S`hodhana (by Virechana,Raktamohshan`a etc.), when the Dosha_s settle in the Tvak and Maamsa OR in condition where the strength of the Dosha_s is mild, in such conditions Baahya (Lepa, Pradeha) Chikitsaa is advised. (C.Chi.21/68-71)

Method of applying Pradeha:

  • Pradeha should be applied frequently, the previous one should be removed and a new one should be applied in such a way that it should be neither thick nor thin. But only in case of Kaphaja Visarpa Pradeha should be Ghana (thick). (C.Chi.21/99-100)

o The Lepa should not be stale (Paryushita).

o The Lepa once removed should not be reapplied. (C.Chi.21/101-102) (A.H.Chi.18/18-19)

o The Lepa should not be wrapped in a cloth and applied. If applied then the heat of the body does not get transferred and hence it causes Svedana, causingPidaka(boils) and Kan`d`oo(Itching) at that site. As a result Kleda and S`hoola also develops in Visarpa. (C.Chi.21/103)

o But Vagbhata is of the opinion that the Lepa should be smeared on a cloth and then applied. (A.H.Chi.18/18-19)

o Lepa should not be applied one above the other, it leads to the same sequences as above i.e. it causes Pidaka,Kan`d`oo, Kleda, S`hoola. (C.Chi.21/104).

o Similarly the Lepa should neither be Ati Snigdha (Over Unctuous) nor Ati Drava (very liquid) in nature as it does not remain on the site and hence cannot pacify the Dosha_s. (C.Chi.21/105). {If the Lepa is too much diluted or watery in nature then it dries off quickly, the Rasa and Veerya of the drug does not reach the Vyadhi Moola as it dries of quickly. (C.Chi.21/106).Similarly if the Lepa is devoid of Sneha (unctuous), it also dries off without giving the desired effect. (C.Chi.21/107)}

Properties of Pralepa:

Kalka of the Dravya used should be applied. Thickness of Pralepa should be 1/3rd of the Angus`hta and it should be neither too Snigdha (unctuous) nor Rooksha (Dry) orPinda (bolus) nor Drava (liquid). (C.Chi.21/68-70)

Granthi Visarpa Chikitsaa:

  • After Oordhva and Adha S`hodhana ,Raktamoks`han`a should be done,
  • Langhana , Rookshan`a Seka ,Pradeha.
  • Followed by Vaata and Kapha Shaamaka Chikitsaa. (C.Chi.21/120).
  • Daaha Karma and S`halya karma is advised here.
  • Treatment similar to Raktapitta should be adopted.
  • Pinda Sveda, Upanaahasveda and
  • Kapha Vaata Naashaka treatment.(A.H.Chi.18/23)
  • Gulma Bhedana Chikitsaa should be adopted.
  • Also Peedana with Ushn`a Loha ,Lavana or Paashan`a is also advised.(A.H.Chi.18/31)
  • If there is no relief found from the above methods then Daaha Karma should be done with either Kshaara, S`hara or by Hema. S`halya karma is advised as a last resort.Paachana karma is also been mentioned by Vaagbhat.(A.H.Chi.18/32-33)
  • Frequent Rakta Moks`han`a, Vaata, Kapha Dosha pacifying measures, Svedana and S`hamana are to be adopted.(A.H.Chi.18/34)
  • Along with the other diets advised the individual suffering from Visarpa should consume Yava,Godhuma or S`haali; Which ever is compatible. S`haali should not be advised in Kaphadhika conditions. (C.Chi.21/114)

Importance of Raktamoks`han`a in Visarpa:

Just as Basti Chikitsaa is considered as Ardha Chikitsaa in Vaata Vyaadhi, Similarly in this context Raktamoks`han`a is considered as Ardha Chikitsa. (C.Chi.21/141). (A.H.Chi.18/36), (S.Chi.17/16)

Vidradhi Chikitsa

Introduction:

  • Vidradhi is an elevation on the surface of the skin which manifests with Oedema. It is S`hastrakarmasaadhyaVyaadhi.
  • Charakaachaarya has explained Vidradhi as one of the types of the PramehaPid`akaa. But he has also stated that without suffering form Prameha, thesePramehaPid`akaa including Vidradhi can get manifested.

Nirukti:

  1. Dusht`aRaktaatimaatratvaat Sa VaiS`heeghramVidahyate |

TatahS`heeghravidaahitvaatVidradhiItiAbhidheeyate | (Cha. Su. 17/95)

  1. S`heeghravidaahaadVidradhih | (Su. Ni. 9/1 Gayadaasa)

Sampraapti:

  • Dosha vitiate skin, Rakta, Maamsa and Medas. They are harboured on Asthi. Gradually, these Dosha create serious Oedema. This Oedema (S`hopha) has large base. There is pain. It is circular or rectangular. This S`hopha is called as Vidradhi.

Types:

  • Baahya :
    • It originates from skin, Snaayu and Maamsa. It resembles to tendons and is extremely painful.
  • Aabhyantara :
    • Dosha when enter interior of the body affecting Maamsa and Rakta (Asra), thereby causes deep seated abscess (Granthi), which is very painful.
    • This Aabhyantaar (internal) Vidradhi may happen at heart (Hridaya), Kloma, Yakrit (Liver), Pleehaa (Spleen), Kukshi (Flanks), Vrikka (kidneys), Naabhi(Umbilicus), Vamkshan`a or Basti (Urinary Bladder).
    • Due to vitiation of Blood (Rakta), the Vidradhi get suppurated quickly, due to this quick suppuration only, it is called as Vidradhi. (TatahS`heeghraVidaahitvaatVidradhiItiAbhidheeyate | (Ch. Su. 17/95)
    • AabhyantaVidradhi manifests same as of Gulma (GulmaRoopin`am)

  • Vidradhi again can be classified as:

  1. Vaatika
  2. Paittika
  3. S`hlaishmika
  4. Saannipaatika

  • Types:

It is of 6 types:-

  1. Vaataja
  2. Pittaja
  3. Kaphaja
  4. Saannipaatika
  5. Kshataja
  6. Asrija

General guidelines to know:

  • When Vidradhi suppurates completely and ruptures, then if Vidradhi is situated at upper side of the body (above the chest), it discharges out through mouth. If Vidradhi is situated at lower side of the body (below the umbilicus), it discharges out through the anus. When the Vidradhi is at umbilicus, after ripening it discharges through both mouth and anus.
  • Completely ripened Vidradhi at Hridaya, Naabhi and Basti and SaannipaatikaVidradhi are incurable. They lead to death. So as soon as a person affects from Vidradhi, one should immediately get treated by Snehana, Virechana therapy.
  • The general rule to treat Vidradhi is like Gulma.
  • It is necessary that Vidradhi should be treated before the complications are manifested.
  • Actually, Pid`akaa related to Prameha and those which are manifesting without Prameha have same line of treatment, as Dosha and Dooshya both are same in these types. D`alhan`a.
  • It has been described that Pid`akaa are manifested due to negligence to treat Prameha. But it is also possible that without Prameha, Pid`akaa can be manifested. Unless and until these Pid`akaa get substantial root in the Dhaatu they are not manifested.
  • S`haraavikaa, Kachchaapikaa and Jaalinee are difficult to tolerate and very serious in nature. They occur in such patients where Medas and Kapha are in excess.
  • Sarshapee, Alajee, Vinataa and Vidradhi are due to dominance of Pitta and are possible in a person having moderately vitiated Medas and Kapha. These are curable.

Incurable condition for Vidradhi:

If Pid`akaa are manifested in a patient suffering from Prameha, at MarmaSthaana, Amsa, Guda (Anal region), Stana (Breasts), Sandhi (Joints), Paada (feet), then this patient does not survive.

Vaataja:

This Vidradhi is black in colour. It is Parusha. There is severe pain. This S`hotha manifests with different ways. This is VaatajaVidradhi. The secretions are thin (Tanu).

Pittaja:

This resembles to ripened fruit of Udumbara. It may be of S`hyaava (gray) color. There is Jvara and Daaha. It manifests quickly. It ripens / suppurates quickly. The secretions are yellow coloured (Peeta).

Kaphaja:

It resembles to an earthen vessel. It is whitish and cold to touch. It is stable. There is mild pain. It manifests with longer duration. It suppurates too lately. There is Kan`d`oo also. The secretions are white colored (Sita).

Saannipaatika:

There are different colored secretions, pain. The Ghaat`aala (The upper part) of the Vidradhi is large in size. It is irregular in size (Vishama). It is Asaadhya.

Kshataja / Abhighaataja:

Due to the trauma there is formation of wound. The heat at the wound gets spread due to Vaayu which provokes Pitta with Rakta. There is Jvara, Trishn`aa,Daaha. This is AagantuVidradhi which manifests same as Pitta Vidradhi.

RaktaVidradhi

  • The Vidradhi is covered with multiple black colored blisters. It is grey colored (S`hyaava). There is severe burning sensation, pain and Jvara. It manifests with the symptoms of Pitta Vidradhi. This is RaktaVidradhi.
  • RaktajaVidradhi manifests in the females only. If there is improper delivery of fetus or if Aparaa (Placenta) is not evacuated properly, then there isRaktajavidradhi which is also called as Makkalla. If this is not pacified within 7 days, then it gets suppurated within seven days.

According to the site of the manifestation, Vidradhi is of 9 types

  1. Gudaja
  2. Naabhigata
  3. Kukshigata
  4. Vamkshan`agata
  5. Vrikkagata
  6. Pleehaagata
  7. Hridayagata
  8. Yakritagata
  9. Klomagata

    • When Pleehaagata, Yakritgata, Klomagata, Vamkshan`agataVidradhi_s burst out, they are drained in the upward direction.
    • Other Vidradhi_s like Gudagata, Bastigata, Vrikkagata, Vamkshan`agata drain in the downward direction.
    • Vidradhi may it be Apakva, Pakva, may it be large or small in size; if it is manifested on the Marma; then it is Krichchhrasaadhya.

Difference between Gulma and Vidradhi:-

  1. There is no root of the Gulma (Nibandhah NaAsti); where as Vidradhi has its root (Sanibandha)
  2. Doshathemselves are turned in to Gulma; in GulmaDosha doe not get harbor in to some other Dooshya; whereas in VidradhiDosha get harboured in to the Maamsa and S`hon`ita.
  3. Gulma is like a bubblein the water which manifest in the Srotas, it resembles to VaataGranthi, therefore it does not get suppurate (as there is no involvement of Rakta and Maamsa in the pathogenesis of Gulma); whereas Vidradhi is having Maamsa and Rakta as Dooshya, it gets suppurated.

AsthigataVidradhi:

Vidradhi manifested on the Asthi causes suppuration of Majjaa. When these suppurated MajjaaDhaatu can not drain out then it causes severe burning pain. Due to this heat produced in the Majjaa, there is severe pain. When this gets drained out, there is unctuous (Snigdha), white colored secretions which is cold to touch.

StanaVidradhi:

In the lactating or non lactating breasts, Dosha cause S`hotha residing in to the StanaSiraa. This manifests in to Prasootaa or Garbhin`ee. The S`hotha isGhana. This manifests as BaahyaVidradhi.

It does not manifests in the girls as the StanaNaad`ee are small in them.

Vatavyaadhi and its Treatment in Ayurveda

Nirukti:

  1. Vaata Eva Vyaadhi Vaatavyaadhih |

Vaata itself is treated as Vyaadhi considering its potential to cause Vyaadhi with dominance of Vaata. Here, Vaataitself acts as a Dooshya and manifests diseases all over the body or locally.

  1. Vaataad Vyaadhih Vaatavyaadhih|

Any disease which is caused due to the dominance of Vaata Dosha, is Vaatavyaadhi.

  1. Vyaadhipadasaamaanaadhikaran`yaad Vikrito Duhkhakaaree Vaato Vaatavyaadhih | M. N. 22.1-4

Introduction:

  • Vaata is main entity as Dosha in the human body. It is the most potent Dosha as many activities of other Doshaare dependent on it. Jvara and other Vyaadhi are also caused by Vaata Dosha also, but they are not denoted asVaatavyaadhi, because other Dosha like Pitta and Kapha both are equally involved, in the pathogenesis of Jvaraetc. diseases. But in the Vaatavyaadhi pathogenesis and manifestation the major role is of Vaata Dosha. OtherAavrita Vikaara like Kaphaavrita and Pittaavrita Vaata etc. are considered under Vaatavyaadhi because there is major involvement of Vaata Dosha than other Dosha.

  • Vaata is Atibala, Aas`hukaaree, Gariyas i. e. it is the most potent and powerful Dosha by which other Dosha; i.e.Kapha and Pitta are influenced. As the Vaatavyaadhi are difficult to cure and have specific treatment, they are described separately. But it is not so with the Pittavikaara and Kaphavikaara so they are not described separately.

  • It is very difficult to describe all the Vaatavyaadhi as there can be innumerable combinations depending upon the site, vitiation of Vaata type etc. Therefore these are to be judged according to the site and the symptoms which are manifested.

  • The prime Dosha in the pathogenesis of Vaatavyaadhi is Vaata, but it is also necessary to find out the association of other two Dosha with the help of the symptoms.

  • Vaata gets aggravated due to two major causes:
  1. Dhaatu Kshaya
  2. Maargaavaran`a

  • Vaata is the most potent Dosha. Being Sookshma (subtle), Vaata moves all over the body and vitiates otherDosha. This leads to the diseases at the site at which the Dosha are aggravated. It also causes emaciation ofDhaatu.

  • Charaka Samhitaa has given the 5 types of Vaata Dosha according to Sthaana (site) and Kriyaa (Vaata responsible for different actions). They are Praan`a, Udaana, Samaana, Vyaana, Samaana, Apaana. When the actions of specific type of Vaata is aggravated or diminished, it is to be taken as a vitiation of Vaata. Actually, 80 types ofNaanaatmaja Vikaaraa are explained in Sootrasthaana of Charaka Samhitaa.

Vatarakta Chikitsa – Treatment for Gouty Arthritis in Ayurveda

For Education & Reference Purpose. Please consult with Ayurveda doctor for treatment

Saamaanya Chikitsaa:

  • Raktamokshan`a: It can be carried out by two ways taking Bala (strength of the patient) and Dosha into consideration.
    • Prachchhaana
    • Siraamokshan`a

Raktamokshan`a should be carried out in Vaatarakta, where there is Raaga, Rujaa and Daaha.

  • Virechana: It is useful to reduce Kapha and Medas
    • Snehana should be carried out first.
    • It should be Sneha Virechana.
    • Virechana can also be carried out with Rooksha Dravya (dry) in a patient in whom Snigdha (unctuous) attribute is increased.
    • Virechana should be mild in nature to avoid the consequent vitiation of Vaata.
  • Basti: Basti should be given repeatedly (Asakrit) as no other modality is as affective as Basti in the management of Vaatarakta.
  • Seka, Abhyanga, Pradeha: These also can be carried out in a patient of Vaatarakta.

Upadhathu Chikitsa

Introduction:

Upadhaatu are the byproducts formed during the process of formation of Dhaatu (body constituent). As they are derived from Dhaatu, they are called as Upadhaatu.

Following chart describes relation of Upadhaatu and Dhaatu.

Dhaatu Upadhaatu
Rasa Stanya, Aartava
Rakta Kan`d`araa, Siraa
Maamsa Vasaa, Tvak
Meda Snaayu
Asthi Danta*
Majjaa Kes`ha*
S`hukra Ojas*

(* According to S`haarangadhara Samhitaa)

According to some Aachaarya; Danta and Kes`ha are Mala of Asthi and not the Upadhaatu. Similarly Ojas is also not an Upadhaatu but epitome of all the Dhaatu.

These Upadhaatu get affected by various causative factors producing signs and symptoms and diseases. Some of the signs and symptoms are very prominent and hence treated as major diseases but some remain dormant and at times participate as one of the factors of Sampraapti Process.

Saamaanya Chikitsaa

While treating these diseases it is advisable to administer all the therapies indicated for vitiation of respective Dhaatu also. E.g. for Stanya Dusht`i the Saamaanya Chikitsaa of Rasapradoshaja Vyaadhi should also be administered along the other line of treatment.

Vis`hesha Chikitsaa

  • Stanya Dusht`i Following should be the line of treatment for Stanya Dusht`i for the feeding mother
  1. Snehana
  2. Svedana
  3. Vamanafollowed by Samarjana Krama
  4. Snehana
  5. Virechanaconsidering Dosha, Kaala, Bala followed by Samarjana Krama
  6. S`heshadosha Paachanaartha Aahaaraand Kalpa

o The child should also be treated in same way but following some precautions.

The treatment of diseases for children should be done similar to adults but the medicines should be mild.

Dose of the medicine should be small (Kaneeyasee Maatraa) as children are delicate (Sukumaara Alpakaaya) and eat small quantity (amount and food items) of food (Sarva Anna Anupasevana).

In Ksheeraada and Ksheeraannaada Baalaka i.e., in breast fed children before weaning, the medicine is applied to the nipple and areola of the nursing mother before each feed.

  • Rajodusht`i This should be treated by treatment similar to S`hukradoshahara Chikitsaa in male.
    • Rajodusht`i should be treated by Snehana, Svedana followed by Vamana, Virechana, Anuvaasana Basti, Aasthaapana Basti and mainly Uttarabasti. The medicines to be used for the S`hodhana therapy should be decided according to the vitiated Dosha.
